In order to apply for a Residency program at University of Sao Paulo Medical School (FMUSP) you must fill in the Orange Form (Click here to access it) and send it to marco_lemonte@fm.usp.br.

Read the instructions carefully:

1. Print the Form

2. Do not fill it out by hand  

3. Fill in ALL fields and date

4. Attach the Registration Form

5. Attach the Registration Statement

6. Attach the Residency Plan

7. Attach your proof of Insurance with the  effective date

8. Attach the Statement of Acknowledgement
